Before Your Visit
You’ll need a photo ID, authorization form(s) from your employer and company-specific paperwork to be completed by the provider.
CDL holders are required to bring their driver’s license.
During Your Visit
If you meet with a provider, they will perform a comprehensive exam to ensure you receive appropriate job placement or treatment.
If you meet with a clinician, they will perform all necessary testing, keeping you informed throughout the process.
After Your Visit
All necessary treatment paperwork and work status notes will be provided to you and your employer in an encrypted, secure method.
Any necessary re-check appointments will be scheduled for you before you leave the clinic.
Please note that the following Occupational Medicine procedures will only be offered daily until 7pm:
Pre-employment and annual employment physicals
Pre-employment drug screens (regulated and non-regulated)
All other occupational medicine services
Exceptions to the 7pm daily cutoff include:
Post-accident, reasonable suspicion and random drug and alcohol screens
Workers’ compensation injury care
WellNow is your single-source provider — with our expansive geographic footprint, we can help employers with multiple locations streamline their occupational medicine program. We will assign a knowledgeable point of contact to your organization to help maintain an understanding of your needs, uphold continuity of service and cultivate a long-lasting partnership.
No. When you set up an account with WellNow Urgent Care, same-day reporting will be custom to your request. We can send results via fax or email.
Organizations do not need a service agreement in place prior to obtaining occupational medicine services for employees. However, we do encourage companies to establish service agreements with us as they help to delineate which services your employees need as well as streamline communication for billing and reporting. With a service agreement in place, we can provide follow-up occupational medicine services for workers’ compensation cases. For walk-in occupational medicine patients whose employers do not have service agreements, payment is required at the time of service.
Yes, our service providers in all of our locations are on the National Registry list and all of our drug screen collectors are DOT drug and alcohol certified.
